Objective: The aim of this study is to assess the value of Intraosseous Infusion (IOI) lines in Extremely Low Birth Weight (ELBW) neonates, as an alternative access to failed peripheral vascular lines in emergency resuscitation.

Design: Review of medical records and office charts of a cohort of 10 ELBW neonates in whom IOI lines were placed when establishment of other venous accesses was impossible. Setting Maternity and Children Hospital (MCH) and King Khalid International Airport Hospital (KKIAH), Riyadh, Saudi Arabia, in the period from June 1994 to July 1997.

Results: All cases survived the primary resuscitation process. One case had mild subcutaneous infiltration that has been resolved spontaneously. There was no long-term sequel among survivors.

Conclusion: IOI provides a rapid, easy, safe and effective vascular access in compromised ELBW neonates, when other Methods fail.

Key words: Intraosseous Infusion; Neonates; Extreme Low Birth Weight in patients with B Thalassemia major & this confirms different other studies.
