A RETROSPECTIVE STUDY OF NEONATAL MORTALITY DURING 1990--2000

Objective: To investigate the influence on neonatal mortality rate (NMR) by setting up neonatal division and neonatal intensive care unit (NICU) in local perinatal center.

Methods: Jiaxing Maternal and Child Healthcare hospital was confirmed as one of the first group “friendly baby” hospital in China in 1992.(1) Investigating and comparing: 17283 newborns during Jan 1,1990 to Dec 31,1992 ,before setting up NICU were enrolled as the group A.32716 newborns during Jan 1, 1993 to Dec, 31, 2000, after setting up NICU were enrolled as the group B. We compared NMR, survival rate of very low birth weight (VLBW) and triplet, quadruplet between group A and B. (2) Investigating NMR and survival rate of VLBW and multiple pregnancy in the process of NICU development. (3) NMR in that locality after the NICU opened to public.

Results: (1) NMR: 8.62 ‰in group A and 3.00‰in group B. Correctional mortality was 6.36‰ and 1.74‰ respectively. X2 =71.33, p<0.01. Survival rate of VLBW was 39.3% in group A, 87.6% in group B. One quadruplet and 7 triplets were all survived after setting up NICU. (2) NMR was 4,51‰ in 1993, the first year of NICU, 3.47‰ in 1994~1997, 1.39‰ in 1998--2000. Correctional mortality was 3.19‰ 2.02‰ 0.49‰respectively. Survival rate of VLBW was 70.8%, 91.2% and 92.5% respectively. (3) Annual NMR in the 5 years after NICU opened to public was 4.02‰~4.24‰in downtown area ,6.46‰~8.32‰ in that suburbs. The NMR of this city was 5.35‰in 2000.

Conclusion: NMR has decreased by 5.62‰since NICU was set up. VLBW survival rate has increased by 48.3%, triplets and quadruplet survival rate are 100%. With development of NICU, mortality of newborn and VLBW reduced progressively. The downtown area’s NMR is in advanced level in China.
