Shanghai Second Medical University, Shanghai, China AIMS: To evaluate the serum
testosterone regulatory mechanism of AMHsecretion at puberty and to
determine the AMH measures in clinicalapplications METHODS: 48 health adolescent
boys in various genitalia Tanner stages fromG1~G4-5 (control group) and 10
male patients aged 12~20 years old with completeGHD and hypogonadotropic
hypogonadism (patient group) were assayed for serumAMH, T, LH and FSH. All
patients were enrolled in Genotropin (0.1U/kg/d) treatment for 2 years with
combined HCG (1000u Biw) or Testorison Depot (100mg/m) treatment in the
2ndyear. Blood samples were collected atstart and thereafter every 6
months. RESULTS: ①In control
group, Serum AMH levels were highest in prepuberty, declined subsequently
according to advancement of pubertal stage, being lowestin stage G4~5
(F=16.78, P<0.0001), and in negative correlation withconcentration of T
(r=-0.49, P<0.0001);②The patient group evaluated serum AMH levels as
318.0±34.7pmol/L at pretreatment, significantly higher than the
normalcontrols (32.75±14.6pmol/L) (t=-8.15, P<0.0001). After the first
year of rhGHtherapy, mean serum level of AMH declined to 195.4±61.2pmol/L
(t=4.01, P<0.005). A further decrease of serum AMHlevels were observed
at the end of the 2ndyear (by combinedtreatment for 1 year), also
significantly different to levels before using sexhormones (t=3.511,
P>0.01). Serum T levels elevated in contrast to thedecreased AMH
concentration. CONCLUSION: ①Serum AMH
levels evaluate negative correlation with serum Tconcentration in normal
adolescent boys.②Serum AMH reveal high values in GHD patients
with hypogonadotropichypogonadism, but decline by the rhGH and sexual hormone
treatment withconcomitant elevation of T concentration.③Measurement
of serum AMH may provide additional functional test fortestis by estimation
of Sertoli cells as well as Leydig cellsfunctions.
