The study on predonisone in preventing renal involvement of children with Henoch-Schonlein purpura

Objective: To investigate the effect of predonisone in preventing renal involvement of children with Henoch-Schonlein purpura (HSP).

Methods: During December 1996 to January 2000, 84 cases of children with HSP whose renal hadn’t been involved were divided into two groups randomly, preventing group (44 cases) which accepted predonisone treatment (1mg/kg.d) for 2 weeks and control group(40 cases) which didn’t receive predonisone treatment. These patients were followed up 1.6±0.7 years.

Results: (1) in control group, 14(35%) of 40 patients developed renal involvement; whereas only 4(13.6%) of 44 patients who accepted predonisone developed HSPN (P<0.05). In HSPN patients of both groups, the 24 hours excretion of protein in urine of control group were very higher than that of preventing group (589.47±148.39mg/d versus 356.28±102.35mg/d respectively, P<0.01). (2) This study also showed that the use of predonisone gave the patients with HSP early could alleviate their abdominal and joint symptoms positively (P<0.05), but it was no effects in preventing these symptoms (P>0.05).

Conclusion: corticosteroids such as predonisone can prevent renal involvement of children with Henoch-Schonlein purpura early.
