THE DIAGNOSIS OF LATE-TOXIC-TYPE BACILLARY DYSENTERY IN CHILDREN

Objective: To advance the diagnosis of late-toxic-type bacillary dysentery.

Methods: To analysis data of 22 dead cases who were diagnosed as bacillary dysentery, toxic type. It includes clinical manifestation, lab findings, intervention and clinical progression.

Results: Most of the cases were infants. The youngest was three months old. There were diarrhea in all cases. Other symptoms included high fever (10 cases), medium or serious dehydration (10 cases), vomited (1 case). bacillus dysentery was found from all children’s stool culture. Seventeen of them were diagnosed as bacillary dysentery, common type at beginning. Five of them were diagnosed as infectious diarrhea at fist. The main complications included pneumonia, mycotic enteritis, septicemia, haemolytic uraemic syndrome. There were fundamental diseases such as malnutrition (16 cases), rickets (5 cases) and anemia (11 cases). Comparing bacillary dysentery, toxic type , the difference point was that the cases occurred in the 5th to 35th day of diarrhea. The reasons of dead included multiply organic failure, respiratory failure and circulatory failure.

Conclusion: The cases have diagnosed definitely as bacillary dysentery, toxic type and those occurred later than bacillary dysentery, toxic type we usually diagnose. We give the name ---late-toxic-type bacillary dysentery. We should pay more attention when the infants suffered from some kinds of fundamental diseases such as malnutrition, rickets and anemia, and get bacillary dysentery at the same time.
