Objective: To explore the possibility of more practical diagnostic criteria
on Kawasaki Disease.
Methods: According to the current diagnostic criteria
of Kawasaki Disease (KD), we retrospectively studied and analyzed the
clinical manifestations including the time and rate of appearance.
Results: 1. 69 admitted cases of Kawasaki Disease were analyzed. In the early
stage of the disease (within 5 days since onset), the incidence of fever,
injected and or fissured lips, skin rashes, bilateral nonexudative
conjunctival injection were 100%, 92%, 84% and 81% respectively. 2. 50 cases were found the
appearance of perianal skin injection and/or desquamation, while most of
them appeared within 7 days of the disease. 3. 11 of 69 cases had discribed BCG inoculation scar reaction
(injection and /or vesicular reaction on BCG inoculation site, "BCG
scar" reaction). 8 of 11 cases presented "BCG scar"reaction
and all appeared within 5 days after onset. 4. 56 cases used γ-immunoglobulin
while 35 cases used in 7days after onset .All patients received aspirin
treatment .
Conclusions: 1. When a baby appears following trilogy during early stage:
fever, bilateral nonexudative conjunctival injection, injected and or
fissured lips (Kawasaki appearance), KD should be highly suspected and γ-immunoglobulin and aspirin should be considered. 2. "BCG
scar reaction" and perianal skin injection and especially desquamation
may be a specific value on early diagnosis of KD. "BCG scar
reaction"is easily ignored by doctors. 3. If trilogy and any other one
signs of KD appeared, the patient should be treated as early as possible.
