Objective: To investigate the treating regimen and prognosis of acute viral myocarditis(AVM) with severe cardiac arrhythmia in 46 children.
Methods: All cases were given a large dose of vitamin C (150 ~ 200mg/kg.d), dexamethasone (0.5~2.0mg/kg.d), antiarrhythmia drugs and other drugs; the patient failed to drugs treatment implanted temporary or permanent cardiac pacemaker.
Results: There were 46 cases with AVM accompanied with severe cardiac arrhythmia. In which 20 cases had III° atrioventricular block (AVB), 5 cases had II°-II AVB, 4 cases had severe sinus bradycardia, 4 cases had ventricular tachycardia, 1 case had atrial flutter accompanied with atrial tachycardia and ventricular tachycardia, respectively, and 11 cases had other severe cardiac arrhythmia. The symptom improved immediately after drugs treatment in 38 cases, but 8 cases failed to drugs treatment and had persistent deteriorating symptom implanted temporary or permanent cardiac pacemaker. There were 40 cases cured completely, 3 cases improved, and 3 cases died.
Conclusion: Implanting of temporary or permanent cardiac pacemaker in the early period of severe AVM accompanied with severe cardiac arrhythmia failed to drugs treatment could improve their prognosis.
