study of medicinal treatment of refractory nephrotic syndrome (rns) in children

Objective: To study the therapeutic effects of single prednisone (pred) treatment and combined treatment in children with RNS .

Methods: 296 children with RNS were randomly divided into four groups. Group A (control) only received oral pred (1.5-2mg/kg/d) at one-day intervals; Group B, C, and D (acting as treating groups) received a combination of oral pred plus IVIG (400mg/kg/d, for 3~5 days),oral pred plus polysaccharide sulfate(PSS 1-2mg/kg/d, for 7~10 days) and oral pred plus intravenous cyclophosphamide (CTX 10mg/kg/d, for 2 days), respectively. The treating groups received the same dose of pred as group A.

Results: After 3-month treatment, the remission rate in group B(75%),C (74%),and D(71%) were 18% higher than that in group A (53%) (p<0.05 or 0.01); but no difference of remission rate among group B,C and D; side effects were observed in 78% of group A, 13% of group B,14% of group C, and 25% of group D. After 12-month treatment ,the remission rates in group A, B, C, and D were 62%, 76%, 89%, and 91%, respectively (c2=26.12,  P<0.001); the remission rates in group B, C, and D were 14% higher than that in group A (P<0.05 or 0.001);and side effects were observed in 58% of group A, 7% of group B, 12% of group C, and 10% of group D(P<0.05).

Conclusion: Combined medicinal treatment obviously enhanced the therapeutic effect on children with RNS and had high remission rate, little side effect and short curative duration.
