文本框: THE RELATIONSHIP BETWEEN IgA NEPHROPATHY AND HBV INFECTION IN CHILDREN

Objective: The purpose of the present study was to clarify the correlation between HBV infection and the pathogenesis of IgAN, and providing the evidence for improving the clinical therapy of IgAN. 
Methods: Thirty-five children aged under 16-year-old with IgAN from Oct. 1992 to Jan. 2001 were studied. Clinical presentations were macroscopic hematuria; microscopic hematuria with or wothout proteinuria; nephorotic syndrome and acute nephritis syndrome. Renal tubuli-interstitium changes were classified into four stages (stage 0 to stage III). HBAg were detected by immunohistochemical techniques in renal biopsy of all patients. HBV DNA was investigated by using in situ hybridization in 8 cases. Three of them with positive HBAg both in sero and kidney tissue were treated with antiviral drug, Lamifuding, 100mg/day p.o. 
Results: The positive rate of HBAg in renal tissue detected by immunohistochemical techniques was 34% (12/35). HBV DNA was positive in 6 of 8 cases by using in situ hybridization. Renal tubular atrophy, interstitial inflammation and fibrosis were more sever in IgAN with HBcAg positive group than that in negative group (P<0.05). The clinical symptoms got great improved in the two cases of IgAN with antiviral drug. 
Conclusion: HBV infection might play an important role in the pathogenesis and progress of IgA nephropathy in children.
