文本框: CLINCAL VALUE OF MP-DNA DETECTION BY THE TECHNIQUE OF FLUORESCENT-QUANTIATION PCR

Objective: To study the clinical value of fluorescent-quantitation PCR technique on Mycoplasma pneumoniae DNA (MP-DNA) detection on children with respiratory tract disease (according to international disease classification code: 446, 493 and 480-486).
Method: 1. Samples for MP-DNA detection were taken from throat swab and or peripheral blood from children with respiratory disease, by the technique of fluorescent-quantitation.2. Clinical diagnosis of MP pneumonia were made when patients had received broad-spectrum antibiotics (two kinds of antibiotics simultaneously, such as penicillin or oxacillin plus augnintin or clafton or rocephin and so on, simultaneously) at least 5 days, while clinical manifestations and signs quickly improved once macrolide (erythromycin or axithromycin or roxithromycin) employed.3. Relationship between the MP-DNA positive results and the clinical diagnosis coincidence rate were analysed by correlation analysis according to the quantitation of MP-DNA.
Results: 1.968 of 2267 cases (42.7%, 1028 samples) were admitted for pulmonary infection from Nov.1999 to Feb. 2001, were detected for MP-DNA. 307 of 968 cases (31.7%) showed throat swab and/or blood MP-DNA positive, 185 cases were clinically diagnosed as MP pneumonia. Mean clinical diagnosis coincidence rate was 60.26%. It takes 19.11% in total MP-DNA positive cases; 2. Statistical analyses showed that MP-DNA quantitation was positively correlated with clinical diagnosis. When MP-DNA quantitation were ~1X102, ~5X102, ~5X103, ~5X104, ~5X105, ~5X106 copies, clinical coincidence rate were 48.75%, 50%, 63.26%, 72.22%, 83.33%, 90% respectively,  (γ=0.98); 3. Near 50% babies below age of 2 with MP frequently represented cough, asthma and moist rales and wheeze could be heard and easily to be diagnosed as asthma and bronchiolitis; 4.Varies chest X ray presentations may occur. Unilateral and bilateral massive lesion or consolidation takes about 15.51% in children under 3 years of age.
Conclusion: 1. Throat swab MP-DNA detection is an simple, reliable and rather sensitive method in clinical diagnosis of MP pneumonia. 2.Clinical diagnostic coincidence rate is positively correlated with MP-DNA quantitation. 3. 50% of patients bellow 3 years of age presented asthma or bronchiolitis appearance.
