文本框: ENDOSCOPIC SCLEROTHERAPY OF ESOPHAGEAL VARICES IN CHILDREN WITH PORTAL HYPERTENSION

Objective: Bleeding of esophageal varices caused by portal hypertension is an uncommon but serious complication in children, Mortality is very high and it lacks of effective method in children. Adult reports the endoscopic injection sclerotherapy is effective in the treatment of bleeding esophageal varices. The aim of this study to investigate the efficacy of sclerotherapy of esophageal varices in children with portal hypertension. 
Methods: 10 children with portal hypertension and esophageal varices bleeding (extrahepatic portal hypertension 5, cirrhosis 4,schistosome infection 1) presenting with hematemesis and /or melena were subjected to esophageal varices sclerotherapy using 1% Aethoxysklerol as sclerosant during 1996-2000.6 .The mean age of the patients was 8.2 years. Male is 9 case, female is 1 case. 9 children had splenectomy before. Gastroscopic examined showed esophageal varices of 10 children and partly erosion with bleeding. 4 case with portal hypertension gastropathy  
Results: 10 patients had a total of 22 injections, 8 had two and 2 had three injections. 9 case give general anesthesia, besides 1 give larephageal anesthesia. Rebleeding after therapy happened only in 1 case. Other 9 patients had no rebleeding. Follow-up examination revealed variceal grade decreased significanty after the first injection, eradication of varices was complete achieved in 4 children. And basically eradication in 5 children 
Conclusion: Our results suggest that endoscopic sclerotherapy of esophageal varices in children with esophageal varices with portal hypertension is a safe and effective procedure. Long-term regular endoscopy follow-up and repeated sclerotherapy are needed to prevent bleeding due to recurrence of esophageal varices.
