文本框: A LARGE DOSAGE CORTICAL HORMONE AND DEMAND VENTRICULAR PACEMAKER (VVI) PLAY AN IMPORTANT ROLE IN TREATING FULMINANT MYOCARDITIS

Objestive: To investigate the important effect of treatment with different dosage cortical hormone in 7 cases of fulminant myocarditis.
Methods: 7 cases were enrolled in the study, 3 males and 4 females; aged from 5-12 year old with an average of 8.5 years old. Clinical symptoms: different degree fever, hypodynamia,pale,annoyance; 4 caseshad Adams-Stokes syndrome.  Electrocardiogram: IIIatrioventricular block (AVB), ventricular tachycardia (VT), sinus arrest, sinus bradycardia, ST-T changes. 3 of them were used by demand ventricular pacemaker (VVI), the rest were treated by  medicine.we used different dosage cortical hormone in 6 cases: normal dosage dexamethasone0.1-0.25mg.kg-1.d-1,large dosagedexamethasone 0.5-1mg.kg-1.d-1and methylprednisolone 15-30 mg.kg-1.d-1. While all cases used antibiotics were used in all cases.
Results: 5 cases lived, 2 cases died. Among 5 surviving cases, 3 were with VVI, 4 received a large dosage cortical hormone treatment. The remaining one with VVI and normal dosage hormone,had sequel of complete right bundle branch block (CRBBB). In 2 dead cases, VVI were not used. One was treated with normal dosage hormone, died of VT; the other was dead on the way to hospital without using cortical hormone. None of living cases got re-infection.
Conclusion: Using a large dosage cortical hormone and VVI in fulminant myocarditis, we can reduce its mortality and improve its prognosis. In order to avoid potential infection of VVI, correct antibiotics would be used in the patients.
