TREATMENT BY ENDOTRACHEAL LAVEMENT CAUSED ATELECTASIS PRODUCED AFTER EXTUBATION OF ARTIFICIAL VENTILATOR

Objective: To observe the effects by using trachea lavement in atelectasis after extubation

Method: Artificial ventilator was used in 158 cases in our NICU from 1993 -1997. 7 cases of atelectasis happened after extubation (4.4%). Endotracheal lavement was used in all cases. At least 2 experienced doctors were present in carried of out of the whole procedure under heart and lung monitor. Trachea tube was introduced through oral carity and approach near carina. Patient was lied on same side of atelctasis, 1-2ml N.S. sdation was injected through the endotracheal tube. Entratrachea suction was performed after adequate blap on the back of the patient: Times of lavement was dependent upon the condition of patient or twice a day.

Results: The clinical standard of convalescence in as following: (1) Disappearance of clinical symptoms, (2) Increment of TcSO2 and PaO2, PaCO2 decreased, (3) Chest film is clear. The range of pH was 7.108~7.256, PO2 was 46-54.7mmHg and PaCO2 was 52.1-92.1mmHg. After lavement the above value is changed to pH 7.235~7.388, PaO258-80.4mmHg, PaCO2 44-51mmHg respectively. Only one time of lavement was taken in 2 patients, and two times in 3 patients, and 3-5 times in another two cases .All the patients will be cured.

Conclusion: Treatment by trachea lavement caused atelectasis produced after extubation of artifical ventilator is safety econimic and effectable also to avoid using vantilator again.
