COMPLICATION PREVENTION OF DUCKETT'S
PROCEDURE IN CHILDREN WITH HYPOSPADIAS

Objective: To assessment the complication of Duckett¡¯s
procedure in 83 children with hypospadias from Feb. 1998 to Mar. 2001.
Methods: In group A 40 children (1.5~12 years old with a mean
of 4.4 years old) were routinely received Duckett¡¯s procedure. In group B
43 children (1.5~12 years old with a mean of 3.2 years old) were treated by
the modified Duckett¡¯s procedure. The characteristic features of the
modified technique were 1) The formation of tubularized island flap and
urethral anastomosis were performed by microsurgical technique. 2) Every
effort should be made to preserve the penis superficial fascia (Colles
fascia) so as to reinforce tube flap and provide a good blood supply. 3) A
little bigger silicon urethral stent to prevent urethral anastomotic
stricture.
Result: In group A urethral fistula occurred in 12 cases,
urethral anastomotic stricture in 2 cases. The success rate has been 70.0%.
In group B all the cases except 2 children complicated by fistula were
satisfactory. The success rate has been 95.35%.
Conclusion: The perfection of techniques and preservation of
Colles fascia can improve success rate and decrease complication.
