Text Box: STUDY OF TOPIRAMATE AS 104 CASES THERAPY FOR EPILEPSY IN CHILDREN

Objective: To study the effect, dose, and adverse events of Topiramate (TPM) for epilepsy in children, either as monotherapy or add-on therapy.  
Methods: 32 patients as therapy of TPM for partial seizures, 47 patients for Lennox-Gastaut syndrome (LGS); 25 cases for West syndrome. All cases were studied as self-controlled, open-label research. The patients were started at 0.5~1mg/kg.d TPM. The dose increased by 0.5~1 mg/kg. d per week. Mean doses were (6.2+2.2) mg/kg. d for partial seizures, and (7.4+3.5) mg/kg. d for LGS,  and (8.1+4.1)mg/kg. d.  
Results: 1) Three groups were treated with TMP for 104 cases, with an average course of 6 months. There were a 50% or greater reduction from baseline in seizure frequency in 80 cases (76.9%), 42 cases (40.4%) no free seizure. 2) There were a 50% or greater reduction in Partial seizures in 26/32 cases (81.3%), 15 cases (46.9%) free seizure. There were a 50% or greater reduction in LGS in 35/47 cases (74.5%), 19 cases (40.4%) free seizure. There were a 50% or greater reduction in West syndrome in 19/25 cases (76%), 8 cases (32%) free seizure; 3) The three seizure type were a 50% or greater reduction in 73.7%, 76.5%, 72%, and 61.5%, 46.2%, 27.2%  free seizure in add-on therapy groups; They were also 50% or greater reduction in 92.3%, 69.2%, 81.8%, and 61.5%,46.2%,27.2%  free seizure in monotherapy ones. 4) The VPA and CBZ serum concentrations of all patients in add-on patients were examined before and after the treatment, but no statistical differences. 5) Negative reactions: 18 patients (17.3%) had bad lethargy, 7 patients (6.7%) had apathy, 3 cases (2.8%) had decreased study ability, 25 patients (24.0%) had bad appetite, and another 8 cases (7.7%) had no sweat when it was hot day.
Conclusions: TPM has proven effective and safe anti-seizure drug as monotherapy and add-on therapy in children partial seizures.
