OBSERVATION ON DUHAMEL¡¯S OPERATIVE THERAPY OF CONGENITAL MEGACOLON

Objective: This report describes Duhamel¡¯s operative therapeutic effects of infant and child¡¯s congenital megacolon.

Material & Methods: We treated 47 cases of infant & child¡¯s congenital megacolon with Duhamel¡¯s operative therapy in recent 10 years.  The oldest is 7 year-old, youngest 2 month-old.  11 cases of minor type; 31 cases of general and 5 cases of major in all 47 cases with Duhamel¡¯s radical therapy under intravenous anesthesia. We can incise the middle transverse colon artery and reserve marginal intestinal one for major type of major involved cases.  For the advanced case or with intestinal enlargement and hypertrophy, we can trim and anastomose them.

Results: None anastonotic leakage, none death in all 39 afterwards-inquiry cases (82.9%), anastomotic stenosis: 2 cases (5.1%), faeces contamination: 3 (7.6%) enterocolitis 1 (2.5%), more than 5 bowel movements per day: 2 cases (5.1%).

Conclusions: It is a good method to treat infant & child¡¯s congenital megacolon with Duhamel¡¯s operative therapy and deserves popularity.  It takes less operative time and less incision, also has better effects in younger than in older one.  There it is advised to treat the case with Duhamel¡¯s method at their earlier age.
