FACTORS BEING RESPONSIBLE FOR PRECARIOUS STATE IN YOUNG CHILDREN

We
have studied 2948 case reports of children at the age under 3 years
retrospectively with purpose to identify factors being responsible for
precarious states for health during the period 1997-1999. Of them 1121(38%)
patients were girls and 1827(62%) - boys. The children had diagnosis of
convulsive syndrome in 203 (6,8%) patients, obstructive syndrome - in
1833(62,2%), toxic encephalopathy in 338(11,46%) and severe form of
pneumonia in 574(19,4%) patients. The special attention was paid to the
premorbid state of children, development of the pregnancy and delivery.
The
presence of the unfavourable premorbid state was characteristic for
children with precarious state. It was found that 38,2% of children had
perinatal encephalopathy, 29,7% - rachitis, 21,9% -hypotrophy, 7,8% -
paratrophy, 34,1% -anemia of the 1st degree, 18,7% - thymomegalia and 17,7%
- exudative-catarrhal diathesis. Analysis of the mother's obstetric history
showed that the infants were born from 1-3 pregnancy and in 32% of cases
the pregnancy was associated with anemia, in 39,5% of cases - with
toxicosis. The deliveries were resolved by Cesarean section in 7,7% of
cases. Of these children 15,2% were born premature, 59,2% - in asphyxia,
24,4% of children had artificial feeding. On the basis of the results of
investigation it may be concluded that encephalopathy, anemia, thymomegalia
and unfavourable obstetric medical history are the factors responsible for
precarious health state in infants. Thus, the retrospective analysis showed
a great importance of the identification and recognition of the factors
responsible for the precarious health state, which allows to develop
differential approaches to the therapy and rehabilitation of this
contingent of young children.
