Text Box: DOUBLE¨CBLIND, PLACEBO-CONTROLLED FOOD CHALLENGE, TEST WHICH SOLVES PROBLEM OF BASELESS LONG MILK FREE DIET TREATMENT Rowicka G, Hozyasz K, Klemarczyk W, Ksiazyk J, Milanowski A Pediatric Department of National Research Institute of Mother and Child Warsaw, Poland Objectives: Allergic reaction to food is the first manifestation of allergic symptomatology in atopic infants. Cow¡¯s milk proteins are the first allergen encountered in large quantities in infancy. Cow¡¯s milk allergy [CMA] symptoms are anaphylactic-immediate, intermediate or delayed- develop several hours or days after milk ingestion. CMA can affect several organs, especially the gut, skin, and respiratory tract. Double-blind, placebo¨Ccontrolled food challenge [DBPCFC] is widely accepted as the most reliable method of diagnosis adverse reactions to food. Material and Methods: Forty six children, 26 males and 20 females 3 to 12 years old, (median age 8,4 y), which were treated with milk free diet because of suspicion of adverse cow¡¯s milk reaction in infancy was observed. Ten children presented in infancy with irritability, 12 - vomiting, 12 - diarrhea, 8 - eczema, 2- growth failure. In 20 children symptoms developed during exclusive breast-feeding. In all adverse symptoms resolved on hypoallergic diet (amino acid or peptide based formula). According to parents cow milk provoked adverse food reactions such as diarrhea, abdominal pain, dermatitis, rhinitis. Parents of all children received a family allowance because of their child disease. Skin prick cow¡¯s milk test was negative in all children. All children were assigned to cow¡¯s milk for a 2- week period under double-blind conditions. During the test they were hospitalized and the observation diary was conducted. Results: During DBPCFC two children demonstrated diarrhea, 4-dermatitis, 2-bronchitis obstructiva. The adverse cow¡¯s milk symptoms were observed only in 8 children/46 (16,6%). Conclusion: Sometimes parents for fear of adverse food symptoms don¡¯t introduce milk to they children diet, but other that fact reason was loose of family allowance. DBPCFC solves problem of baseless long milk free diet regimen. 2328
