RESULTS OF SURGICAL TREATMENTS IN CHILDREN AND YOUNG ADULTS WITH DILATED CHOLEDOCHAL CYST (DCC)

The aim of our work is to provide results of surgical treatments in children and young adults with DCC. For past 6 years in our clinic we have seen 14 patients with cystic transformation of the common bile duct. Out of these patients, 5 were operated elsewhere: cholecystoduodenostomy side to side was done to 3 patients; cystojejunostomy-1, hepaticojejunostomy on isolated small intestines Roux procedure-1. In 11 cases operation done was- incising the cystic changes of the common bile duct with creation of hepaticojujenostomy end to side on isolated small intestines Roux procedure. To two of the patients hepaticojujenoduodenostomy on isolated isoperistaltic segment of the small intestine with anti reflux invagination as adopted in our clinic. All the patients were discharged in satisfactory clinical conditions and were followed up. The post- surgical observation was from 10 months to 5 years. To these, 9 patients had good results, while 3 with satisfactory results. Unsatisfactory results were seen in 2 patients with development of chronic reflux cholengitis. Later on to these patients anti reflux correction was performed.

 

Therefore, experience showed that to these groups of patients, the initial surgical correction must be considered with anti reflux prophylaxis.
