EXPERIENCE OF TREATING BILIARY COMPLICATIONS IN CHILDREN AND YOUNG ADULTS

The objective of our research is to share the experience we have in treating complicated biliary system in children and young adults. In our clinic from 1995 to year 2000 we admitted 18 children with complicated biliary system. 11 of them with mechanical jaundice in echinococcose of the liver, 3- after surgical correction due to cystic transformation of common bile duct, 2-post traumatic lesion of the bile ducts (1-iatrogenically, 1-post traumatic rupture of the supraduodenal part of the common bile duct due to blunt abdominal injury), 2- secondary to tumor of the pancreas. Patients with echinococcose of the liver, echinococcectomy was done to them, the resulting cavity was closed and the bile fistula sutured. In 6 cases cholecystectomy was additionally performed with drainage of the common bile duct T- tube. Patients with trauma and cystic transformation of the common bile duct hepaticojujenostomy with transhepatic drainage of the bile ducts-Saipol procedure was done. A patient with adenocarcinoma of the pancreas that can not be operated we did cholecystoduodenostomy side to side anastomosis. To a ten years old patient with cystoadenoma of the head and body of the pancreas pancreatoduodenal resection was done. Post operatively feature of bile fistula formation was seen in one patient with satisfactory end. Therefore, different reasons leading to complications of biliary system are relatively rare in children which requires extra care of the patients in specialized surgical units.
